New Enrollment

After 30 days of being on assignment you will be auto-enrolled into our Blue plan benefits program.

Enrollment date is the first Monday following 30 days*.

If you do not want to enroll, you must waive your coverage.

* Coverage begins on the first Monday following 30- days of employment. The date of employment is based on the date of your first check.

Yearly Enrollment

You will be automatically enrolled in the Blue Plan for the new year even if you have previously waived coverage.

How do I add a dependant to my plan?

Call 833-236-7463 and speak with an enrollment specialist. There is no limit to the number of dependents that you can add.

How does a Teammate turn in medical bills if they receive them and have insurance?

Call 833-236-7463

What is the typical delivery time for insurance cards for new enrollees?

You will be automatically sent ID cards when you enroll in our benefits plan. Please make sure that we have your correct address on file so that these cards go to the right place. Once you receive them, please create your UMR account as soon as possible, as this will make replacing the cards, finding a provider and other things much easier.

How to create your UMR account and access digital Medical and Dental ID cards

Step 1: Visit http://www.umr.com
Step 2: Click on Create your account
Step 3: Select Member from the dropdown of what type of user you are
Step 4: Click on Register
Step 5: Enter the following:
- First and last name, date of birth, Member ID, and group policy number (76416533) in order to create a HealthSafe ID.*
- Then, click Continue and follow the remaining steps to create your HealthSafe ID username and password
Step 6: Use your newly created HealthSafe ID credentials to log in and access nearly all of your health benefits information!

* You may use your Social Security Number if you do not have your Member ID card available.

I don’t understand why some of my insurance claims are being denied. Where can I go for help?

There can be many reasons for this, from coding errors or non-qualified services. Please call the Teammate Benefits line at 833-236-7463 and a representative will help you with your concerns.

What kind of PTO do teammates get?

Teammates that qualify for Vacation pay have to take the full 40 hours at once. They can turn it in and continue to work the same week or actually take the vacation time off.
Holiday Pay: Teammates are eligible for holiday pay after 1,200 hours. You will be paid for six holidays (New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day). In order to be eligible for the holiday pay, you must work 1,200 hours during the 12 months preceding an Ōnin paid holiday. You must also work the scheduled days before and after a holiday.
Vacation Pay: After each 1,800 hours of service with no more than a 120 day break in assignments, you will be eligible for 40 hours of vacation time. If your employment with Ōnin is terminated, you will not be paid for any accrued vacation time
